Our Promotion of Access to Information Act, 2000 (PAIA) Section 51 manual summary — how to request access to records we hold. Last updated: 14 July 2026.

In short: under PAIA, you (or your authorised representative) have the right to request access to records held by Sampax Health 911, including your own medical/patient records. This page tells you how.

2. Purpose of this manual

This manual is published in terms of Section 51 of the Promotion of Access to Information Act 2 of 2000 ("PAIA"). It explains the categories of records we hold and the process for requesting access to a record — including access to your own health/patient record, which you are entitled to request in terms of both PAIA and the National Health Act.

3. Records we hold

We hold, among others: patient clinical/medical records; appointment and booking records; medical-aid claims and billing records; staff and practitioner registration records; supplier and contractor records; and general correspondence records. Some of these records are automatically available (e.g. our published service information on this Website) and do not require a formal request.

4. How to request a record

To request a record (for example, a copy of your own patient file), contact our Information Officer using the details in Section 1, providing: your full name and contact details; sufficient detail to identify the record requested (e.g. your patient reference, date range, or visit dates); the form in which you'd like access (copy, inspection, electronic); and proof of identity. If you are requesting on behalf of someone else, you must provide proof of authority to act for that person.

There is generally no fee for a data subject accessing their own basic health record; a reasonable request/access fee as prescribed under PAIA regulations may apply for other records or bulk copying.

5. Response timeframes

We aim to respond to all requests within 30 calendar days as required by PAIA. If a request is refused in full or in part, we will provide written reasons, and you have the right to lodge a complaint with the Information Regulator or apply to court for relief.

6. Grounds for refusal

Access may be limited in narrow circumstances set out in PAIA — for example, to protect the personal information or health information of a third party, legal privilege, or where disclosure would unreasonably endanger someone's life or safety. Any refusal will state the specific section of PAIA relied upon.
